- Kupuna care programs involving intensive case management designed to maintain independence and quality of life for as long as possible. Support services for seniors include transportation, group lunch program, home delivered meals, light housekeeping, laundry assistance, meal preparation, grocery shopping, yard work, pet care, installation of assistive living devices, personal care, home health care and family caregiver support (training, support group).
- Provide assistance with applications for TANF (Temporary Assistance to Needy Families); Subsidized Housing; Food Stamps, Medicaid and QUEST; and Medicare Supplemental Programs.
Patient Charges and Billing System
Hana Health provides health care services to all people regardless of their ability to pay. Services are provided according to a sliding fee scale which is based on federal poverty guidelines. In order to determine whether patients are eligible for services at a discounted rate, they are required to provide financial information to Hana Health including household size and household income. Eligibility for service discounts must be determined before the care is provided. Once Hana Health obtains this information, a determination is made about the patient’s eligibility for discounted services according to federal requirements. All patients are required to pay a minimum fee of $10.00 for medical and dental visits. This fee is waived for children and adolescents under 18 years old.
Discounts are provided for:
- Primary health care including physical exams, laboratory services provided on site, x-rays provided on site, treatment of chronic medical conditions, treatment of illness, immunizations and mammograms provided off-site.
- Primary dental care including exams, cleaning, x-rays, fluoride, dental sealants, fillings, and extractions.
- Medications that are prescribed by Hana Health physicians only and are obtained at Hana Health.
- A limited amount of outside laboratory tests.
For seniors who have Medicare, discounts are also applied to all required co-payments.
Transportation
For Hana residents who do not have transportation from their home to the Health Center, Hana Health will make arrangements to provide patient transportation for scheduled appointments. Transportation arrangements can also be made for health care appointments in Kahului and Wailuku when referred by Hana Health physicians.
